Advanced Diagnostic Techniques Used in the Evaluation of the Impotent Male


Abstract
The history and physical examination have been the principal traditional means of establishing a diagnosis of impotence and of determining whether a psychological or organic disorder exists. However, several advanced diagnostic techniques have been recently developed in an attempt to clarify and classify the etiologic factors. These tests include psychological screening, laboratory studies, nocturnal penile tumescence monitoring, penile blood pressure and pulse measurements, penile angiography and cavernosography, intravenous radioisotope penography, bulbocavernosus reflex latency test, artificial erection, and penile thermography. Herein many of these methods were reviewed and the strengths and weaknesses of each in the overall evaluation were discussed. It is clear that no test has been adequately evaluated or validated as truly distinguishing organic from psychogenic impotence. In general, the individual tests considered alone provide very little information, but each may be useful when considered in the context of the complete evaluation.
